Prediction of Antidepressant Efficacy from Response to the First Ect

چکیده

1. Gupta et al have proposed a 9% depression attenuation separator to differentiate potential responders from potential non responders. This separator is small and is intuitively not logical; for example, a 10% figure would have made m&re sense. The choice of a non-intuitive figure suggests that the separation between responders and non responders was very narrow. In other words, response to the first ECT may not clearly distinguish potential responders from nonresponders. Next, by changing the definition of response, one patient shifted from the good to the poor response category. This altered the statistical significance of the results and confirms that there was poor separation between good and poor responders based on 9% response with the first ECT. Finally, if the authors had extended the ECT course to beyond 6 treatments, some patients classified as poor responders might have responded. These patients would probably have shown less than 9% response to the first ECT. With the addition of these patients to the good response group, the first ECT response construct would have been further weakened.

Titrated moderately suprathreshold vs fixed high-dose right unilateral electroconvulsive therapy: acute antidepressant and cognitive effects.

BACKGROUND The antidepressant and cognitive side effects of right unilateral (RUL) electroconvulsive therapy (ECT) are reported to depend on the magnitude of the electrical stimulus relative to the seizure threshold. The stimulus doses explored in previous clinical trials of RUL ECT have generally been limited to 1 to 2.5 times the convulsive threshold and the antidepressant efficacy has been l...

Dose-Response Relationship of Clinical Effi-cacy and Side Effects of Electroconvulsive Therapy

Background: The electrical dose selected for electroconvulsive therapy (ECT) must have an acceptable efficacy and no or minimal cognitive side-effects. We evaluated the clinical efficacy and cognitive side-effects of ECT in relation to the stimulus dose administered. Method: This study assessed 71 depressed patients who were treated with bilateral ECT. For evaluation of depressive and cognitive...

Comparison of Therapeutic Efficacy of Ect and Imipramine : a Randomized Controlled Trial

Efficacy studies comparing ECT and tricyclics in depression have had methodological limitations. This study compared EC T and imipramine (IMN) prescribed as the first line of treatment in major depression, Drug-naive, consenting, DSM-IV major depression patients (n=28), were randomized to receive either bilateral ECTs or IMN (225 mg/d) for four weeks. Severity of depression was scored at twice ...

Antidepressant effects, of magnetic seizure therapy and electroconvulsive therapy, in treatment-resistant depression.

Major depression is a common mental health problem and associated with significant morbidity and mortality, including impaired social and physical functioning and increased risk for suicide. Electroconvulsive therapy (ECT) is highly efficacious in treatment-resistant depressive disorders, but cognitive side effects are frequently associated with the treatment. Magnetic seizure therapy (MST) is ...

Electroconvulsive therapy--efficacy and side-effects.

BACKGROUND Efficacy of electroconvulsive therapy (ECT) and duration of associated side effects is uncertain. We wanted to study indications, efficacy, time to response and side effects. MATERIAL AND METHODS ECT-protocols and medical records (from the period 1960-95) in three psychiatric wards of Dikemark Mental Hospital, Norway were systematically assessed. RESULTS 141 persons underwent 241...
